The files are from multiple sources and represent either my personal works or files that are freely available on the Internet. If possible, the file's source is cited.

  1. St. Louis University Mental Status (SLUMS) Exam
    A small mental status exam that is reported to be more sensitive than the MMSE.
  2. The Geriatric Depression Scale-Short Version [on-line]
    "This scale was developed as a basic screening measure for depression in older adults. We will consider this page to be under continuous development and try to keep translations of the scale available."

  3. The Functioning Everyday with a Wheelchair (FEW) Seating-Mobility Outcomes Measure
    This site provides information and background on the development of this self-report tool as well as instructions for the administration, scoring, and interpretation of the tool.

  4. Berg Balance Scale
    A well-known and highly used assessment of balance.

  5. Wheeled Mobility Evaluation
    A 3-page evaluation developed to assess clients' medical necessity for powered mobility. It is not designed for severely challenged individuals requiring highly customized mobility needs. NOTE: The evaluation is 3 separate images contained in one zip file. Zip and image viewer programs are required to see these files.

  6. Senior's Fall Risk Assessment
    A 10 page evaluation developed by Broomfield Senior Fall Prevention Program and Home First.

  7. Home Safety Checklist
    Use this list to identify fall hazards and accessibility issues of the homeowner and family members. This checklist can be used by a therapist or given to family members to report on home conditions or to prepare for a family member's discharge to home.
  8. Short Blessed Form
    "A well-studied test, which is (so far) little used. It has been validated against neuropathology, and was derived from the longer Blessed scale."
  9. CMS 700 Form
    The CMS 700 is Medicare's plan of treatment for outpatient rehabilitation.

  10. Occupational Activity Score Record - Generic Form
    A very simple form that provides quantitative data for occupational activity. There is zero evidence supporting the accuracy of this document, but I find it helpful.
